We ask that payment be made at the time of visit. For your convenience we will submit the appropriate insurance forms electronically at the end of your child’s visit for processing. There will be no need for you to address or mail forms. You will leave our office knowing that the appropriate forms have been submitted. Please make yourself aware of what services and how much your insurance will reimburse you for. We are providers to various insurance plans. Make sure that the specifics of your plan are readily available to determine if the plan you have is one that we participate in and what the coverage is. Keep in mind that insurance companies do not have a code or compensate our office for all the services that we provide as specialists. You will be personally responsible for those additional costs. Orthodontic treatment is an excellent investment in the overall dental, medical and psychological well-being of children and adults, and financial considerations should not be an obstacle to obtaining this important health service. Our desire is to make orthodontic treatment convenient and manageable for you.

The fees for treatment depend on the complexity of the orthodontic problem and the type of appliances used. We will establish a fee and discuss payment options before treatment begins.

If your dental insurance covers orthodontic treatment, you will receive the benefit of reduced personal costs directly to you. Many insurance policies have a lifetime orthodontic maximum that is distinct from regular dental insurance. We will be happy to work with you to understand your orthodontic insurance plan. Please contact the dental insurance company or health care administrator to determine eligibility and reimbursement. The insurance benefits that you will receive often times, in conjunction with our very reasonable fee schedule enables treatment to be affordable for all children and adults

We understand that unique financial circumstances may arise which require a more creative approach to financing. Often a simple phone call can help to answer any questions you may have.

We ask that payment be made at the time of service.  As a courtesy to our patients, we will submit all necessary documents and information to expedite the reimbursements by their insurance company.  Presently we are an in network provider for the Cigna Radius Network and will be happy to work with all other insurance policies.  However, the policy must allow the subscriber to go out of network.   It is the patient‘s responsibility to know if their policy allows them to go out of network.  Benefits can be used as partial payment toward fees incurred.  All balances are due after the insurance benefits have been applied toward the patients account. It is important to note that the patients’ insurance policy is a contract between the patient and the insurance company not between Dr. Lubin and the patients’ insurance company with the exception of the Cigna Radius Network.  It is our hope to enable you to receive the highest level of oral health care.
